Suicide Hotline: What Calling The National Prevention Lifeline Is Like

Springfield, Ill. Mental Health Mental health staff are on call 24 hours a day to talk to people struggling with suicidal thoughts, substance abuse or other mental health issues.

Until recently, the hospital's call center was manned by nurses and other nursing staff. But sometimes when everyone is tied up with patients, calls to him go unanswered, pedaling the caller to the nearest call center, often a state or national call center.

"Staff will answer the phone 24/7, but if they're busy with meals or with residents ... then they won't be ready to take a call," said Diana Naebe, president and system administrator. Memorial Behavioral Health.

Statewide, Illinois-based call centers answered 1 in 5 calls to Lifeline in the state in the first three months of 2022. Another 80% was sent to other states. It's unfair to leave so many calls unanswered, but without state or federal support, Knaebe said it's the best they can do.

The center has already received funding from the federal government to improve its call center. It is part of the rollout of a new national mental health emergency number, 988, which campaigners hope will make it easier for more people to connect with the National Suicide Prevention Lifeline when it launches on July 16.

Knaebe said the new grant allows Memorial to transition to crisis line staffing with dedicated staff instead of hospital staff. Although they still have several open positions they are trying to fill, she said they have already seen a big improvement in their response rates.

"In [April] where we did that, we were able to answer 80% of calls versus 20% in previous months," Knaebe said.

The new crisis line is expected to send more calls, and that means states like Illinois have a tough hill to climb, said Andy Wade, executive director of the Illinois Group of the National Association on Mental Illness.

"You're building on the platform of the suicide prevention line, which is the first point," Wade said. "But it's been an under-resourced one in Illinois for so long that it's not enough to just pick it up."

While other states are dedicating funding to pro-life, Illinois has not followed suit, leaving call centers to rely on private donors and grants.

That's important because the National Suicide Prevention Lifeline automatically directs calls to the nearest call center, and if no one answers, people can wait to be put on hold while being transferred, said Rachel Bhagwat, NAMI's Chicago policy director.

"And that applies to mental health counselors. I mean, when you're talking about someone in crisis or suicidal thoughts," she said.

If someone in Illinois gets in touch with a crisis counselor in Missouri or other states, "How can that person understand what a crisis plan or a mental health tool might look like at home?" Bhagwat said.

John Draper, president of the National Suicide Prevention Lifeline, agrees that calls are being better answered in the state. But he said that the National Center is ready to deal with the flood.

"I can't promise that nobody will wait," Draper said. "I can definitely say that if you hold back, you will be answered and you will be answered by a counselor who cares about your behavior."

New 988 Hotline Is The 911 For Mental Health Emergencies

So mental health experts say addressing mental health needs must go beyond call center staff and long-term treatment options.

Federal funding to help increase staffing at call centers includes $282 million from the USA Rescue Plan, although only $105 million of that is earmarked specifically for hiring.

But some worry about sustainability: Most of the money allocated so far will run out in a few years.

Mental health advocates in Illinois say they want to see the state step up and invest in 988 programs, and they need lawmakers to agree on a consistent long-term funding base. But so far it has not happened.

To help with recruiting, Mindful Behavioral Health consults overtime and hires people who are still working toward a degree or certification.

"The state hasn't really invested in the call center department," said Knaebe, who once led the Illinois Department of Human Services' division of public health. They did it, they felt they wanted to be a part of it. So we did some temporary work."

National Suicide Prevention Hotline Launches In July

Marissa Kollias, spokeswoman for the Illinois Department of Public Services, said in an email that the state has allocated about $4.5 million in federal funds to be used by call centers over the next two years. Additional state funding from cannabis tax revenue and other federal sources results in about $7.5 million in annual spending on call centers. Additionally, the state has a "$71 million obligation for ongoing crisis management, including mobile crisis response support."

Kollias also said the reason for the low response in the state at the moment is "because most of our local governments don't have a call center to cover them." Only 37 of Illinois' 102 counties have a call center contracted with Lifeline to answer calls in the state.
